Progress Reports Due
September 30, 2021
Hello Community Partner,
It’s an honor to be working with you to support the Washington State Department of Health’s COVID-19 vaccine communications campaign. The end of September will be the midpoint for this effort so we ask you to complete a brief progress report by September 30, 2021. This report gives you an opportunity to share more about your organization and the recent activities you’ve accomplished with this funding. DH will use this information to spotlight your organization in public-facing reports.

If you have any technical issues with the form, need assistance in completing this report, or need the report in a different format, please contact your outreach coordinator at DH or simply reply to this email.
Please note: A final report will be due November 30, 2021 unless you have been notified otherwise. DH will communicate about the requirements of this final report by November 15, 2021, however, you can expect it will be focused on specific components about engagement and community/audience reach, uploading final deliverables, sharing feedback from your community and sharing personal insights and learnings from doing this work.
