As event spaces, restaurants, workplaces and healthcare facilities attempt to operate as safely as possible, proof of vaccination has become a common requirement for patrons, employees and patients. The Washington State Department of Health (DOH) created WA Verify, a digital COVID-19 vaccine record, to help Washington residents easily prove their vaccination status.
How It Works
WA Verify uses the state immunization registry to match your name and phone number or email with your COVID-19 vaccination information. It then produces a QR code you can easily screenshot or download to a phone or other electronic device. This QR code can be used as proof of vaccination anywhere it is required.
Get Your WA Verify COVID-19 Vaccine Record
To get your digital COVID-19 vaccine record, visit https://waverify.doh.wa.gov/ and enter your name, birthday and phone number or email address. You will be asked to enter a 4-digit PIN of your choosing before selecting “submit.” Within 24 hours, you will receive a text or email from WA Verify requesting your 4-digit PIN. Enter your PIN and your digital COVID-19 vaccine record will appear. Screenshot or download the record to your device(s), and use this anywhere proof of COVID-19 vaccination is required.
