We are inviting all community partners to complete a brief survey to give feedback about your experiences with the Washington State Department of Health’s (DOH) Community Media Outlet (CMO) COVID-19 vaccine communications program.
To complete the survey, please visit this link and submit your responses by June 20. The survey should take you no longer than 10 minutes to complete.
Why take the survey?
This survey will help DH and DOH understand the successes and challenges of the DOH CMO program and help improve public health communication partnerships in the future.

Anyone interested in having further discussion about your experience as a community partner is also invited to participate in a 45-60 minute insight interview with DH.
